  DonorPass Questionnaire

DonorPass Saves Time! Completing a health history questionnaire is an important part of the blood donation process that is required by the Food and Drug Administration (FDA). You can now use DonorPass to complete the health history questionaire before arriving at one of our participating donor centers listed below.

Once completed, you will print off your DonorPass, which has encrypted barcodes similar to a printed airline boarding pass, and bring it with you to your donation. Upon arriving at a participating donor center, the staff will review your health history questions using your DonorPass.

Where can I use DonorPass?

Donor Pass is currently accepted at all community donor center locations. A complete list can be found here.

DonorPass is now available for use at select Mobile Drives.

We are in the process of implementing DonorPass at our mobile drives. You should have received an email indicating that your blood drive will accept DonorPass. If you are unsure if your mobile drive is accepting DonorPass, please check with your group coordinator or chairperson before you begin.

**Important** The FDA requires that the online health history questionnaire be completed on the same day as your donation (beginning at 12:01 am) and that all questions be aked for each donation. If you plan on donating today and would like to use Donor Pass, please begin by clicking here.
